Many hospital outreach lab programs are dealing with the double whammy of successive reductions in Medicare Part B Clinical Laboratory Test fees and a shrinking budget by their parent hospitals
Over the past 24 months, all the painful cuts to lab test prices have made depressing news. These decreases in the prices paid to clinical laboratories by the Medicare program and major private health insurers mean that labs in 2014 will have significantly less revenue, even as the volume of specimens to be tested increases.
The logical response by many medical laboratory organizations has been to take active steps to decrease expenses. In fact, the price and revenue erosion experienced by labs of all sizes and types over the past two years is the reason why cutting unnecessary costs is now a primary management goal at clinical laboratories throughout the United States. Where hospital margins to be squeezed, that would place hospital laboratories under greater budget constraints
Hospitals are honing in on Medicare’s new value-based purchasing program quality metrics in an effort to improve patient care—and earn reimbursement rewards. Clinical laboratory managers and pathologists will want to track implementation of this program, because it is one further step forward in Medicare’s plan to move away from fee-for-service reimbursement.
As part of its effort to drive quality improvement at U.S. hospitals, the Centers for Medicare and Medicaid Services (CMS) issued final rules in 2011 for the first year of its Hospital Value-Based Purchasing Program (HVBP). The program is a pay-for-performance initiative that begins in fiscal 2013. Modern Healthcare reported on this story.
“[The HVBP structure] has been very eye-opening to a lot of people because we are not used to being compared that way,” observed Jeff Costello. He is Chief Financial Officer at Memorial Hospital & Health System in South Bend, Indiana. This 526-bed institution is on the latest Thomson Reuters’ 100 Top Hospitals list.

DATELINE: San Antonio, Texas—This week, a large and enthusiastic crowd of clinical laboratory managers and pathologists gathered here to participate in the fifth annual Lab Quality Confab, a conference devoted to Lean, Six Sigma, and process improvement in medical laboratories and hospitals.
It was an opportunity to judge the “state of the clinical laboratory testing industry” in regards to how quality management and the tools of Lean and continuous improvement are helping to transform the nation’s more innovative medical laboratory organizations. This is an important trend in laboratory medicine and it is still in its earliest stages.
